EST Talks

The EST Talks will be held in the evening, between 17:30 and 21:30. They will be divided into 20-minute sections: Expert Talks or Round Tables.

The Expert Talks will be a 20-minute talk given by a worldwide expert in the field. Moving away from the 8 minute/talk format, with a very wide spectrum and a very narrow benefit, will increase the value of the talks for the audience. The subject will be a very narrow part of the respective field and it will be covered by an expert in that field - a person who conducts research or has extensive expertise in that particular topic.

The Round Tables will be 20-minute Q&A sessions moderated by local faculty regarding controversial or extremely impactful topics in the fields of acute care surgery, emergency medicine or critical care. The questions addressed to the experts will be selected by the moderators after engaging with the public (Social Media sessions in the months preceding the meeting will request questions for the experts from our community).

The EST talks will have the following precise goals:

  • to impart extremely useful knowledge to clinicians, knowledge that can be used the next day to save lives.
  • to highlight recent, practice changing progress in a field; to show the value and impact of recent publications.
  • to inspire and drive forward personal or institutional change that will lead to better care for our patients.
  • to show the beauty of emergency medicine, acute care surgery and critical care.

Description:
The Emergency Surgery Course® is a joint initiative of the European Society for Trauma & Emergency Surgery (ESTES) and the American Association for the Surgery of Trauma (AAST). ESC® aims to aid surgeons in non-trauma emergency surgery by providing the know-how and the decision-making skills necessary for daily practice. The course consists of theoretical sessions, case scenarios and state-of-the-art videos in an interactive format.

– Hone decision-making skills pre-, intra-, and postoperatively

Mastering the Abdominal Compartment Syndrome EUR 200 EUR 150

Description:
The abdominal compartment syndrome is a condition with a difficult diagnosis and an even more difficult treatment path. The diagnosis involves critically ill patients with conflicting clinical pictures, difficult or uninformative clinical examinations, and special techniques. The treatment is often frightening, leaving clinicians with few options other than decompressive laparotomy. Mortality for patients that develop abdominal compartment syndrome is well over 40%. As most of the patients at risk for developing this condition are critically ill patients that are admitted to the ICU, the decision to initiate treatment and the type of treatment to be used is a shared decision between critical care physicians and surgeons.

The workshop aims to give participants a complete knowledge of the following:
- risk factors for the development of the abdominal compartment syndrome
- prevention of the abdominal compartment syndrome
- intra-abdominal pressure measurement techniques
- pathophysiology of the abdominal compartment syndrome and diagnostic criteria
- treatment options for the abdominal compartment syndrome
- decompressive laparotomy - technique, temporary abdominal closure and management of the open abdomen

The practical part of the workshops will teach participants to measure the intra-abdominal pressure and to successfully manage the open abdomen.

Description:
The workshop aims to help clinicians use imaging to make decisions for trauma patients that have visceral injuries. Participants will be exposed to clinical scenarios and the accompanying imaging studies. The guided discussion that will ensue will give the clinician the basic tools to understand the exams required for each scenario, to interpret the findings in a clinically meaningful way and to make the correct decisions.

The workshop is aimed at clinicians - emergency medicine, acute care surgeons, critical care physicians. The standard barrier between radiologists and clinicians should be broken for better outcomes. Cooperation is key for success and this can only be achieved by speaking a common language.

Experts will guide participants through the whole process and highlight key points and clinical pearls along the way. The aim of the workshop is to teach basic imaging semiology for trauma scenarios and to understand how that guides the clinical course of the patient.

Excellent Presentations Save More Lives Than a Trauma Surgeon EUR 150 EUR 120

Description:
The problem with medical presentations is that they fail. Simply put, they do not achieve their intended purpose, which is to educate. This is not for a lack of intention or a failure of the presenter to put in the effort. The reasons are "conformity to perceived standards, aspiration to the level of mediocrity displayed, education, defensive practices against performance anxiety, time pressures, a lack of value in the output and ultimately lack of alternative approaches" as Ross Fisher states. He also observes that we know what is wrong with them and we desire improvement but, when actually presenting, "we follow the practices of failure despite effort and care".

Ross Fisher will clarify these aspects and, in his humble words, he'll only offer you the truth. He will, however, also offer you a roadmap to better presentations. And if this is not much in your eyes I promise you that this workshop will change how you think about communication in all its aspects: presenting cases, presenting papers, educating juniors.

Focused Assessment with Sonography for Trauma EUR 150 EUR 120

Description:
FAST is an ultrasound protocol used in trauma patients for diagnosis and triage. Having all the advantages of ultrasound in general (speed, replicability, non-invasiveness) it is perfectly suited to offer quick information for traumatized patients. Its ease of use has led to the FAST exam becoming one of the adjuncts of the primary survey and being integrated in trauma algorithms in many practice guidelines.

While controversies about its use in mature trauma practices exist, the FAST exam, undeniably, offers the following benefits:
- it is a simple, widely available tool that can offer information in austere environments or in mass casualty situations.
- it teaches basic ultrasonographic knowledge that can be used in many other scenarios, outside trauma; the ultrasonographic windows of the FAST exam are integrated parts of other ultrasonographic protocols.
- it has a definite role in diagnosing cardiac trauma and is gaining importance in the decision-making process regarding resuscitative thoracotomy and blunt traumatic cardiac arrest.

Participants will be able to reliably perform a FAST exam in their practice the minute the course is over. Participants will also receive a curated list of papers and literature to further their understanding of the FAST exam.

Mastering Laparoscopic Techniques in Acute Care Surgery EUR 200 EUR 150

Description:
Laparoscopic surgery has a significant and increasing role in emergency surgery. It also has the benefits generally associated with minimally invasive surgery - quicker recovery, less parietal trauma, a lower risk for intra-abdominal adhesions.

The range of emergent procedures laparoscopy is suited for is not as extensive as in elective settings - the distinction between procedures that are suited for laparoscopy and procedures that are not is an important decision. The pendulum swings more and more towards laparoscopic surgery but the choice of minimally invasive procedures should not be detrimental to the patients. At the same time, these procedures require a specific skill set that is only partially overlapping the skills developed during elective surgery.

This workshop will have the following teaching objectives:
- participants will understand when minimally invasive surgery is appropriate
- a case-based discussion will highlight the moments when minimally invasive surgery is no longer appropriate and conversion to open surgery is warranted
- participants will learn skills for laparoscopic appendectomy
- participants will learn skills for laparoscopic perforated peptic ulcer repair
- participants will learn skills for small bowel manipulation (running the bowel and creating a working space)

The goals of this workshop are to make participants comfortable with decision-making regarding laparoscopy in urgent settings and to teach the skills necessary to approach acute care surgery with minimally invasive techniques.

Ultrasound Guided Procedures - A Vital Skill in Critical Care EUR 200 EUR 150

Description:
Ultrasound guidance is increasingly used for a variety of procedures in emergency departments, operating rooms and intensive care units. From placing iv catheters to biopsies and peripheral nerve blocks, ultrasound makes these procedures safer and increases their success rate.

The workshop has the following learning objectives:
- familiarizing participants with biopsy devices
- familiarizing participants with the basic ultrasound equipment used for ultrasound guided procedures
- needle tracing using ultrasound
- performing safe needle access in vascular structures
- performing safe ultrasound guided cavity drainage
- performing safe ultrasound guided biopsies.

The workshop focuses heavily on the practical part. After a short introduction participants will go through guided exercises that will gradually enhance their skills. Tutors will observe and guide the participants and expose them to different procedures and different ultrasound machines and settings. The formula of this workshop has been tested in local and European meetings and provides a high degree of skill acquisition and gives participants the knowledge and confidence to start using ultrasound in their daily practice.

Chest Drainage Essentials: Simulated Training for Real-Life Scenarios EUR 200 EUR 150

Description:
Pleural drainage is an essential skill in emergency medicine, acute care surgery and critical care. It is one of the most common procedures performed in emergency departments and ICUs. It is useful for diagnostic and therapeutic purposes. Whether done formally or using a Seldinger technique it can be one of your best friends when you're on-call.

The workshop will teach pleural drainage in its various technical methods and will also highlight the special considerations for different indications: tension pneumothorax, simple pneumothorax, hemothorax, empyema, and simple pleural effusions in various medical conditions.

Aside from the techniques for insertion, the workshop will teach the basic physics of underwater seal drainage, diagnosing and troubleshooting drainage system malfunctions, and cues for escalating care or removing the drain.

The workshop will heavily feature the practical side of things, with a clear aim to teach participants to safely and efficiently insert a chest tube and connect it to a drainage system. Every step will be guided by expert tutors with hundreds of procedures in the backpack.

Point-of-care Ultrasound: the ICU and ER Game Changer EUR 200 EUR 150

Description:
The use of ultrasound in the Intensive Care Unit and Emergency Department has multiple benefits. In life-threatening circumstances it can offer quick answers to many clinical questions. The patient does not need to leave the ICU or ER and is examined by the clinician - the person most invested in the case and the person that has the most information about the patient. This is a prime example of Point Of Care UltraSound - a clinician performed diagnostic procedure that delivers timely and valuable information. Clinicians that are versed in ultrasound can find other applications outside diagnosis: ultrasound guided drainage, peripheral nerve blocks and ultrasound guided catheter placement.

Participants will learn:
- basic knobology and ultrasound physics
- basic semiology
- the RUSH protocol
- the BLUE protocol
- integrating the information obtained in clinical diagnostic and management protocols

Resuscitative Surgery: Mastering Damage Control Techniques EUR 200 EUR 100

Description:
Damage control surgery can be described as "the great equalizer". It avoids technically challenging and time-consuming procedures for rapid hemorrhage and contamination control. However, even if it does avoid difficult procedures, damage control surgery is not simple. It has a specific skill set that is not found in other branches of surgery - for efficient damage control surgery certain techniques, clinical pathways, and postoperative care protocols must be mastered. Participants will learn decision-making in emergent traumatic situations and understand when to use damage-control techniques and its core components.

During the workshop participants will learn the following techniques, all components of efficient hemorrhage control:
- trauma laparotomy and thoracotomy;
- simple techniques for vascular repair; placing vascular shunts;
- hemorrhage control from solid organ injuries; pelvic packing;
- exposure of great vessels;
- contamination control;
- temporary abdominal closure.

The mindset and techniques used during damage-control surgery are essential for an acute care surgeon. A clear understanding of these skills saves lives and gives a young surgeon building blocks for technical excellence.

Description:
Airway management is a vital part of critical care, anesthesia and emergency medicine. This course will teach participants the technical skills required to manage a difficult airway (awake intubation, video laryngoscopy, intubation with a flexible fiberscope, surgical airway) and will also focus on the human factors of difficult airway management by exposing participants to simulated clinical scenarios.

Managing Hemorrhagic Shock - The Value of the Golden Hour EUR 150 EUR 120

Description:
Hemorrhagic shock is the leading cause of death in the first three hours after trauma. A combination of bleeding control and good resuscitation will give patients the greatest chances for survival. Good resuscitation, however, is not easy to achieve. It is a very complex choreography that implies little room for mistakes and requires quick thinking and quick decision-making.

The elements that this workshop will cover are:
- pathophysiology of hemorrhagic shock
- damage control resuscitation
- goal-directed resuscitation
- trauma coagulopathy
- fluid management during the first hour

Understanding these principles of care for the early management of hemorrhagic shock is essential for all clinicians - they apply not only to trauma patients but can occur in any type of patient, on any medical or surgical ward. Our reaction to this state of shock will have a very big influence on patient outcomes.
